Patient Case Coordinator
Location: Remote (South Africa Support Team)
Employment Type: Full-Time (4pm-12am)

Role Purpose
The Patient Case Coordinator ensures smooth, compassionate, and efficient coordination of all patient care cases. This role acts as the primary point of contact between patients, families, caregivers, and the care management team ensuring each patient receives the right services, at the right time, with consistent quality and communication.
Key Responsibilities
- Patient Intake & Assessment
- Handle all new patient inquiries via phone, email, or online platforms.
- Collect medical history, care needs, preferences, and risk factors.
- Assist with initial screening to determine suitable care plans.
- Care Plan Coordination
- Schedule and assign caregivers based on patient needs, location, and skillset.
- Maintain updated care plans and communicate changes to families and caregivers.
- Coordinate transitional care for post-hospital or rehabilitation discharges.
- Communication & Support
- Serve as the main remote contact for families, patients, and medical partners.
- Provide regular updates, follow-ups, and reassurance to families.
- Handle concerns, escalations, and urgent care requests professionally.
- Case Management Administration
- Update patient records and care notes in the CRM/Home Care Software.
- Monitor compliance, incident reports, and safety concerns.
- Ensure all documentation meets company and healthcare standards.
- Caregiver Support & Scheduling
- Manage caregiver rosters, shift confirmations, and schedule changes.
- Assist caregivers with questions, care instructions, and urgent needs.
- Ensure coverage during emergencies or last-minute changes.
Required Skills & Experience
- Experience in healthcare coordination, patient administration, medical assistance, caregiver scheduling, or a related field.
- Strong customer service and communication skills.
- Ability to work in a fast-paced, high-empathy environment.
- Proficient in CRM or care management software (training will be provided).
- Highly organized with excellent follow-up and case-tracking abilities.
- Ability to multitask, problem-solve, and stay calm under pressure.
- Reliable internet connection and dedicated remote workspace.
Preferred Qualifications
- Healthcare, social work, nursing, or allied health background.
- Experience in elderly care, home care, or patient support roles.
- Understanding of medical terminology and care planning.
