An exciting opportunity has arisen for an experienced older adult clinician to join the BaNES Older Adult Community Mental Health Team, a well-established, multi-disciplinary, integrated team.

The post is based in Bath and North East Somerset, a UNESCO World Heritage Site. It is a beautiful city, surrounded by countryside, with diverse arts, culture, and sports. It's easily accessible from surrounding areas, with rail links to Bristol and beyond. Staff parking, pool cars, electric car charge points, and cycling lock-up facilities are available.

You will join the team during a transition to the key worker role, replacing care co-ordination. Your responsibilities include providing specific interventions to individuals with significant mental health needs and those with dementia, aiming to enhance well-being and independence. This involves collaborative, holistic, and solution-focused work, including delivering therapeutic interventions, supporting treatment engagement, risk assessment and management, care needs assessment, and facilitating appropriate care packages.

The role offers opportunities to develop supervisory skills by supervising qualified and unqualified team members.

KEY RESULT AREAS: Clinical Practice

1. Conduct holistic assessments of individuals' needs and risks following referral.
2. Manage a caseload using the integrated CPA process, acting as care co-ordinator.
3. Establish well-being plans with service users and carers, including evidence-based interventions and outcome measures.
4. Commission appropriate services based on funding and referrals.
5. Regularly evaluate and adjust individual plans.
6. Apply principles and processes per ICPA policy and act as care co-ordinator when needed.
7. Undertake specialist assessments, e.g., cognitive testing or Care Act assessments.

Record Keeping & Administration

1. Maintain accurate, contemporaneous mental health records per policies and guidelines.
2. Input data and statistics for clinical activity reporting.
3. Use time and resources efficiently.
4. Participate in meetings, presenting complex cases as appropriate.

Clinical Liaison

1. Communicate effectively within AWP and with external agencies.
2. Engage sensitively with colleagues, service users, and families.
3. Operate within safeguarding policies and participate in investigations as needed.

Continuing Professional Development and Practice

1. Maintain and develop professional knowledge and skills to ensure evidence-based practice.
2. Follow professional guidelines and Trust policies.
3. Stay informed about medication management and related issues.
4. Meet post-registration education and development requirements.
5. Participate in supervision, appraisal, and staff development.
6. Update skills in IT systems relevant to the role.

Teaching, Education, and Audit
