Heart Failure Specialist Nurse

Hi, I'm Angela and I am the Clinical Lead for the cardiac service. If you're looking for a rewarding role that allows you make a positive impact to the lives of patients with heart failure, come and join our team! This job requires a balance of specialist knowledge and compassionate care which makes the role challenging and fulfilling for cardiac nurses.

We are looking for a Heart Failure Nurse who has experience in looking after patients with heart failure to join our established and experienced community service. The post holder will work within a highly skilled team to support and manage patients with complex Heart Failure, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions.

Main duties of the job
  • To Manage a caseload of patients with complex Heart Failure, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions
  • To work as part of the multidisciplinary team with the aim of preventing admission, supporting effective discharge and providing ongoing care for patients with Chronic Heart Failure within the community
  • To provide expert support, advice, and education to raise awareness of the need of the patient with Chronic Heart Failure
  • To Undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
  • The post holder will be an active team participant within the service and provide in-depth clinical knowledge.

Job responsibilities

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

  • Overall managerial responsibility for caseload of chronic heart failure patients within the service, acting as an expert practitioner and overseeing care carried out by other members of the HF/CR nursing team, providing advice where required.
  • Responsible for providing expert advice on chronic heart failure issues to a wide range of people.
  • Responsible for policy development within own specialist area.
  • Work with the Clinical Manager to ensure planning of the workforce and future of heart failure service provision.
  • Assess patients with Heart Failure referred from both Primary and Secondary Care providers. Discuss suitability for discharge and/or medical management with appropriate medical staff according to agreed protocols
  • Plan, implement and evaluate individualised specialist patient care programmes based on evidence-based practice and the application of clinical reasoning skills. Formulate a self-management plan where appropriate.
  • Ensure that standards of care and treatment are maintained, in line with National Guidance (NICE) European Guidelines, local guidelines and policies
  • Undertake review and monitoring of patient medications, informing GP/Consultant as appropriate.
  • To initiate clinical investigations, interpret results and take clinical decisions based on information available.
  • Prescribe, uptitrate/down titrate patients medications in accordance to the patients clinical response to medications and reviewing the blood test results
  • Communicate effectively and work collaboratively with multidisciplinary team to ensure a co-ordinated service. This will include service meetings, case conferences, prevention of admission and discharge planning meetings
  • Provide advice to patients and professionals and liaise with members of the Neighbourhood Team for provision of palliative care packages
  • Promote better understanding of cardiac conditions for patients, their families and carers, and other agencies involved with the patients through education, support and counselling.
  • Support the Heart Failure Rehabilitation Programme, ensuring appropriate staff skill mix and cover. Make clinical decisions and liaise with MDT on appropriate patient criteria for inclusion into the programme
