Job description

A Vacancy at Manchester University NHS Foundation Trust.

We have an exciting opportunity to recruit a band 3 Trainee Assistant Practitioner / Band 4 Qualified assistant practitioner to join our home pathway service in North Manchester as part of Manchester Community response. To apply you must already be in receipt of your Foundation Degree in Health and social Care (Assistant Practitioner) or already enrolled on the course and working towards this. On successful completion of the 2-year course you will move into a band 4 position.

If you are a qualified Assistant Practitioner you would start at band 4.

We are seeking to appoint a highly motivated and enthusiastic candidate, preferably someone who has previously worked within the rehabilitation setting, studied or is studying the Foundation degree in health and social care, and you must be competent in IT systems. A full UK driver’s licence and access to a vehicle is essential to commute between patient visits.

The post is part of our Manchester Community Response Service which also includes the Crisis Response team, Home Pathway, Crumpsall Vale ICT unit, Reablement, Community IV team, Pharmacy and Social care.

The post holder will work as an integral part of the Intermediate Care Therapy team (and the wider MCR service) to provide a high quality patient‑centred approach to adults in the community in North Manchester.

The team’s integrated service delivery model aims to avoid admissions, reduce length of stay and improve the patients’ experiences through access to the right intervention at the right time, delivered by the appropriate health or social care practitioner.

The aim of the post is to rehabilitate and improve the function of patients by providing interventions in their own home either following discharge from an acute hospital setting or following illness or injury at home.

After completing appropriate competencies, you will be expected to manage your own caseload of patients in the community.

You would be part of a wider multi‑disciplinary health & social care team who work towards regaining or maintaining independence and quality of life. The post holder will have an allocated caseload of patients under the supervision of a registered professional.

The role will also involve team/office based duties to ensure effective service delivery.

The team is committed to continual professional development with regular supervision and appraisals. This post is supported by B5, B6 and B7 Physiotherapists and Occupational Therapists.

The role will also involve close working with student therapists.
