We have an exciting opportunity for registered mental health nurses to join our Early Days in Custody Team (EDiC) at HMP Swaleside.

1. Holistic – the EDiC team is multi-disciplinary, drawing on expertise from other healthcare teams to provide a knowledgeable and skilled service with the ability to draw upon specialist input rapidly when needed.
2. Curious – the EDiC team will engage with patients with clinical curiosity, using all available information to understand patient needs. The team will engage with patients’ families or carers to fully understand their needs and help maintain family relationships.
3. Prioritising Safety – keep prisoners safe by identifying and addressing immediate risks such as self-harm, suicide, and unmanaged substance misuse dependencies.

• Support new prisoners during their first 14 days in prison to access relevant healthcare services and manage their health and wellbeing.
• Identify patients' wider needs through comprehensive assessments and collaborate with them to create a care plan addressing their health and social needs.
• Cover some duties for the mental health in-reach team, including:
• Care planning
• Caseload reviews
• Administering antipsychotic depot medication
• Collect and synthesize clinical history to ensure safety, continuity of care, and accurate records, minimizing re-assessments.
• Ensure ongoing care with clear pathway management understood by patients and their families/significant others.
• Share information appropriately with patient consent to ensure all supporting staff, including HMPPS staff, understand care plans and risks.
