Job description
Senior Physiotherapist - Admission Prevention Team

The post holder will work within the A&E department based at the Princess Royal Hospital. The post holder will act as an ambassador for Sussex Community NHS Foundation Trust and play a key role in supporting at the front door of A&E in preventing hospital admissions. Utilising their knowledge of the community services and clinical skills the post holder will work closely with the A&E staff to ensure seamless, safe and effective discharge plans are in place for the person to return back to their usual place of residence as soon as possible, preventing admission to hospital. The post holder will play a key role in using data from incidents, feedback and observation to identify potential improvements or changes within the service which will support effective safe discharge. To treat all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.

Main duties of the job

The post holder will have excellent clinical decision-making skills that will support the A&E staff in the management of discharging patients into the community and community beds. The post holder will support with the induction of new staff. The post holder will attend MDT meetings, and support A&E staff with complex discharge functions as required. To provide training and education to A&E staff around what support community services can provide and support in discharge management & processes.

To ensure records and data are maintained in a timely manner and in line with information governance and data sets required for the service.

Job responsibilities

To work effectively as a team member in providing and facilitating a high standard of care. To plan therapy, social and rehabilitative pathways taking account of the fluctuating needs of the patient to assist in discharge and prevent hospital admission.

To offer advice and support and education on healthcare to both patients and carers. To use clinical expertise to be able to support A&E staff in the patient pathway to facilitate rapid discharge and prevent hospital admission. To undertake a holistic assessment of all the patients needs and understand the routes for escalating issues that are not directly resolved by own professional practice.

Formulate and deliver an individual treatment programme based on a sound knowledge of evidence-based practice, to ensure a safe discharge and prevent hospital admission.

To evaluate patient progress, reassess and alter treatment programmes as required. Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

To be responsible for maintaining personal accurate and comprehensive patient treatment records in accordance with SCFT Information Governance and professional bodies. To be responsible for maintaining and developing own competency to practice through CPD activities and maintain a portfolio which reflects personal development. To work collaboratively with the Integrated Discharge Team (IDT) and A&E staff in supporting discharges and the flow of patients through the acute floor, to prevent hospital admission.

Person Specification
Qualifications
  • Professional qualification such as a degree or equivalent in Physiotherapy
  • Current registration with the Health and Care Professions Council as a Physiotherapist
  • Evidence of relevant postgraduate courses / CPD and reflective practice relevant to clinical speciality
Experience
  • Substantial relevant post-qualification experience covering a broad range
  • Experience of supervising junior staff, assistants and students
  • Experience of independently managing a clinical caseload
  • Contributions to audit and evaluation within a clinical governance framework
  • Experience of a range of caseload management and prioritisation systems
Skills and knowledge
  • Demonstrates competency in a range of clinical skills
