Overview

An exciting opportunity for an experienced Physiotherapist has arisen in the Haverhill Integrated Neighbourhood Team. The team are based in a Health Centre in Haverhill. The Haverhill Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care so we are able to work together in a fully integrated way.

The successful applicant will join a friendly team of therapists, community nurses and social care staff. You will be providing Physiotherapy to patients in the community and have the chance to work in an autonomous manner carrying your own caseload of patients with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. The role includes provision of therapy for patients with long term conditions but you will also be providing a responsive, same day service to patients to facilitate hospital discharges and carry out joint health and social care assessments of need in the community.

Responsibilities
  • Provide Physiotherapy to patients in the community and manage your own caseload with support from a wider multidisciplinary team.
  • Work autonomously while collaborating with hospital and West Suffolk Alliance teams.
  • Deliver therapy for patients with long-term conditions and provide a responsive, same-day service to facilitate hospital discharges.
  • Carry out joint health and social care assessments of need in the community.
  • Develop and maintain high-level assessment skills across a range of conditions.
  • Supervise and mentor junior staff and students; participate in ongoing learning and development within the team.
  • Contribute to service improvement initiatives in collaboration with the Quality Improvement team.
  • Travel to rural locations as required.
Qualifications and requirements
  • Qualified Physiotherapist with relevant registration.
  • Experience in neurological conditions, musculoskeletal conditions, frailty, falls, dementia and orthopaedics is advantageous.
  • High level assessment skills and the ability to work with patients in the community.
  • Ability to travel to rural locations; a part-time post is available.
  • We operate a 7-day service within core hours 8am to 6pm.
