A rural NHS Trust in England is looking for an experienced Physiotherapist to join their Haverhill Integrated Neighbourhood Team. The role involves managing a caseload and providing physiotherapy to community patients while working as part of a multidisciplinary team. An exciting opportunity for an experienced Physiotherapist has arisen in the Haverhill Integrated Neighbourhood Team. The team are based in a Health Centre in Haverhill. The Haverhill Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care so we are able to work together in a fully integrated way.
The successful applicant will join a friendly team of therapists, community nurses and social care staff. You will be providing Physiotherapy to patients in the community and have the chance to work in an autonomous manner carrying your own caseload of patients with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. The role includes provision of therapy for patients with long term conditions but you will also be providing a responsive, same day service to patients to facilitate hospital discharges and carry out joint health and social care assessments of need in the community.
