Drake Medical Alliance Primary Care Network comprises 5 GP Practices across Plymouth, serving over 58,000 patients.
We are seeking an innovative and enthusiastic Clinical Pharmacist to join our supportive Pharmacy Team, which includes a Lead Clinical Pharmacist, Pharmacy Technician, 5 Clinical Pharmacists, and 5 Pharmacy Technicians.
Main duties of the job
Clinical practice:
- Manage patients with single or multiple medical issues requiring medication optimisation.
- Conduct clinical medication reviews with patients.
Medicines management in practice:
- Provide telephone support for patients regarding their medicines.
- Reconcile medicines post-discharge from hospitals, intermediate care, and into Care Homes, collaborating with patients and community pharmacists.
- Respond to medicine-related enquiries from GPs, staff, healthcare teams, and patients, offering solutions and follow-up.
- Refer patients to appropriate healthcare professionals promptly.
- Contribute to repeat prescribing policies and develop new medicinal services.

We are a well-established primary care network with five GP practices covering Plymouth from Woolwell and Roborough in the north to Wycliffe and North Road West in the south.
Our Pharmacy Service team seeks a Clinical Pharmacist to join our motivated and supportive team.
Job responsibilities
The pharmacist will work within their competencies across DMA, providing expertise in medicines management, conducting structured reviews, managing long-term conditions, transfer of care, and safer prescribing systems. Responsibilities include managing repeat prescriptions, acute requests, and addressing public health and social care needs.
The role involves medication reviews, especially for polypharmacy in older adults, care home residents, and those with multiple conditions. Leadership in quality improvement, clinical audits, and some aspects of the QOF will be expected.
Main duties include:
- Providing clinical reviews for patients with complex needs, making recommendations for medication improvements.
- Collaborating with care home staff and visiting patients at home for medication reviews.
- Monitoring medicines associated with hospital admissions and readmissions.
- Offering patient-facing clinics for medication queries and concerns.
Medicines management duties:
- Telephone support for patient medication questions.
- Reconciliation post-discharge with collaboration from community pharmacists.
- Responding to medicine-related enquiries and providing follow-up.
- Managing repeat prescriptions and ensuring appropriate monitoring.
- Contributing to development of new medicinal services and treatment pathways.
Person Specification
Knowledge and Skills
- Deep therapeutic and clinical knowledge, evidence-based healthcare principles.
- Understanding of social, economic, and environmental health determinants.
- Knowledge of primary care prescribing, rational prescribing, and improvement strategies.
- Proficiency with IT systems, including word processing, email, and internet research.
- Skills in planning, managing, and reviewing medicines for long-term conditions.
- Ability to analyze complex information and identify risks.
- Excellent communication, negotiation, and conflict management skills.
- Ability to motivate, influence, and work collaboratively with GPs and other agencies.
- Accountability for own role and responsibilities.

Experience
- Experience with common acute and long-term conditions in general practice.
- Experience in partnership working and relationship building across organizations.
- Experience with audit, research, project management, and home visits.
Qualifications
- Bachelor or Master of Pharmacy.
- Registration with the General Pharmaceutical Council, up-to-date.
- Understanding of practice standards and policies.
- Possibly working towards or holding an Independent Prescribing qualification.
- Possession of a Clinical Certificate or Diploma in Primary Care and Community.
