A local council organization in Manchester is seeking an Assistant Team Leader to support community health and care services. This role focuses on team management and ensuring high standards of care for service users. Medical Clearance – Pre-placement health assessment (PPHA)
Casual Car Documentation – The successful candidate will be a designated Casual Car User. Car journeys may be necessary on an occasional basis. If you drive you must hold a full, valid driving licence, provide your own car for use at work and give all necessary documentation. You will receive an allowance per mile of business use.
DBS (Disclosure and Barring Service) clearance – This post is covered by the Rehabilitation of Offenders Act 1974. Candidates must apply for an Enhanced DBS check. Information will be kept confidential and is in accordance with the DBS Code of Practice.
Security and Identity Checks – Proof of identity including a recent photograph, full employment history, and evidence of conduct in previous employment relevant to health or social care or children or vulnerable adults.

This is an exciting opportunity to join the Reablement Team as an Assistant Team Leader, providing support to our communities to ensure care is delivered safely and to a high standard.
As an Assistant Team Leader you will actively contribute to the development and management of the Reablement Service, working with stakeholders to achieve citizen outcomes and high standards of care, mindful of community expectations.

You will assist the Reablement Team Leader to deliver effective support to a team of staff, ensuring the smooth day‑to‑day running of the service and effective use of resources to maximise service delivery. You will undertake a range of relevant risk assessments prior to commencement of service to ensure the required provision of care can be delivered safely for both citizens and staff.
As a Reablement team Leader you will adopt a person‑centred approach and will co‑ordinate and develop plans to increase independence and raise aspirations for service users.
You will carry out assessments to determine service users’ needs and provide appropriate support to ensure that outcomes are achieved within required timescales, working collaboratively with partner agencies and stakeholders to ensure that service users are able to access community and national services to achieve their identified outcomes.
