Tamworth Community Health is seeking a passionate and culturally responsive Registered Nurse to join our Integrated Chronic Care for Aboriginal People Program (ICCAPP) team. About the Role
In this unique community outreach position, you will work in partnership with Aboriginal chronic care patients, their families, carers, and the broader community to deliver culturally sensitive, person-centred care. Your goal will be to support patients to manage chronic illness and improve their overall health and wellbeing by achieving identified health outcomes through coordinated, flexible care.

Key Responsibilities

• Provide compassionate, culturally safe, person-centred care in collaboration with the multidisciplinary team.

• Support patients at risk of or living with chronic disease, both in the community and post-hospital discharge.

• Work closely with Aboriginal Health Workers and community organisations to deliver coordinated care.

• Use your strong community connections and understanding of Aboriginal health to advocate and support improved health outcomes.

• Participate in ongoing networking and education opportunities through local services and the Aboriginal Health Unit.

About Our Team
You’ll be joining a small and dedicated ICCAPP team based at Tamworth Community Health, consisting of a Clinical Nurse Consultant, an Aboriginal Health Practitioner, and a Registered Nurse. Together, we deliver a practical and collaborative approach to improving health outcomes for Aboriginal and Torres Strait Islander people aged 15 years and older.
