Overview

Community Neuro Rehab Team Brighton and Hove – Full Time Band 6 Community Stroke Nurse 37.5 hours.

Would you like to develop your nursing skills to work as a stroke nurse? We have an exciting opportunity for a nurse with an interest and the relevant experience to join our Community Neuro Rehab Team, based in Brighton and Hove. You will be part of a large, friendly, welcoming, dynamic multidisciplinary team, delivering neuro rehabilitation to patients in a community setting. You will have supervision from the band 7 stroke nurse and work in the team with Physiotherapists, Occupational Therapists, Speech and Language Therapists, social worker, Psychologist and Rehab support workers.

The role is for a B6 nurse post of 37.5 hours a week with occasional Saturday working.

Responsibilities
  • Delivery of an effective stroke nurse service to community based neurological patients – predominantly stroke. Treating all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences.
  • To practice as a stroke nurse in the community, liaising with the multidisciplinary team and delegating work to rehab support workers or other team members as appropriate.
  • To assist the multidisciplinary team in preparing intervention plans, implementing rehabilitation and evaluating outcomes.
  • Blood pressure (BP) management is a significant part of this role, supporting patients and their families, GPs and MDT colleagues. An additional part of this role is supporting patients and their families to understand what has happened to them in terms of their diagnosis. The stroke nurse role will be expected to offer guidance and education around improving any positive lifestyle changes to empower patients to reduce their risks of further strokes happening.
  • To work jointly with other professionals to ensure a needs based, patient-centred “what matters to me?” approach.

Job Summary

The post holder will be a member of a specialist interdisciplinary rehabilitation team working with patients with acute and long term needs in the community that are neurologically related. The post holder will be expected to provide for the client group by assessing and managing health and rehabilitation needs liaising with primary care, the acute sector, allied health professionals and social care. Health promotion around stroke and the provision of information and education in the community is a significant aspect of the nursing role.

Communication and Working Relationships
  • To communicate with other agencies across the Trust, primary care, voluntary and private sector to support patients and carers / family to maintain and enhance their well-being in the community
  • To participate in coordinating and chairing goal planning meetings and reviews with patients in their homes and within the MDT
  • To communicate effectively with the MDT the outcome of assessments and patient reviews
  • To work in partnership the post holder will communicate effectively with patients and their carers / family, the primary health care team and other stake holders on a range of complex health matters
  • To communicate with patients and carers / family in a format that is consistent with their level of understanding, culture, background and preferred way of learning
  • To be a positive role model for CNRT and to act as an ambassador for CNRT at all times
Key Responsibilities
  • To contribute to the application of evidence-based practice to enable to improve, sustain and promote health and well-being for
  • To work without direct supervision as an autonomous practitioner in the community
  • To support the ongoing development of multi-professional rehabilitation and management of stroke patients, managing acute and sub-acute discharges
  • To work to agreed contracted level of service agreement
  • To assess risk to client, carers / family and support workers when handing over specific rehabilitation / care management programmes in the community; provide specific training to carers and generic workers to support the ongoing programme in the community; reviewing and grading programmes as required
  • To respect the patient’s dignity, wishes and beliefs through the process of shared decision-making, and to obtain their consent for all health and nursing interventions
  • To participate actively in the development and review of own work against KSF outline for the post including the identification of personal development needs and providing evidence for NMC revalidation
  • To contribute to training and education of the multidisciplinary team to ensure team members are competent in areas of shared practice
  • To review current research and evidence-based literature demonstrating use of findings in relevant clinical practice
  • To demonstrate and share best practice for stroke rehabilitation to nursing staff within the team and across SCFT
  • To deliver nursing care as a lone worker in the community demonstrating safe practice; competency in initial risk assessments; emergency procedures and autonomy in decision making regarding patient assessment, intervention and care planning
  • To undertake clinical decisions within the scope of professional practice and to demonstrate personal accountability for actions or omissions in practice, justifying decisions
  • To adhere to the NMC Standards of Conduct as a registered nurse
Main Tasks
  • To plan, develop, implement and evaluate programmes to promote optimum health and well-being of patients and their carers, including appropriate health assessment tools. To be patient and family driven – helping to set and achieve attainable ‘goals’
  • To carry out nurse specific assessment in community settings; assessing risk to patient and carer; identifying impairments in order to establish a patient centred goal specific rehabilitation programme, including medication management, nutrition and hydration, blood pressure management, psychological care and health promotion
  • To offer opportunities to talk about healthier lifestyle options to empower patients and their families to reduce risks of further illnesses (e.g. hypertension/stroke)
  • To carry out initial multi-disciplinary initial assessments and communicate to the team to identify MDT input, needs and risks
  • To be jointly responsible for overall day-to-day management of stroke patient referrals
  • To maintain accurate and contemporaneous records of care ensuring nursing compliance with NMC, and SCFT standards.
  • The post holder will ideally be expected to have a mentor qualification (or work towards) and teach and support programmes of care to persons, registered/unregistered staff, formal and informal carers, third sector colleagues and students
  • To undertake supervision and line management of junior staff and students
  • To provide clinical, informal supervision and training to support workers involving shadowing, joint working input to clinical reasoning and reflective practice as part of the MDT
  • To participate actively in senior clinical meetings and service governance meetings with the team
  • To lead and participate in audit activities towards development and effective MDT working using support from line manager
  • To demonstrate effective time management skills
