The post holder is required to work within an MDT focusing on the older person living in the community who suffer with a wide range of mental health problems. Please note for Welsh students, you must apply via Streamlining where all available jobs can be located.

The post holder will be responsible for assessing clients’ needs, planning and implementing the care package alongside other professionals within the team. They will provide education, knowledge, guidance and support to student nurses and unqualified staff in the team and provide support to clients and their families. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

In conjunction with other team members, the care coordinator is responsible for a designated caseload of clients with both organic and functional mental health problems. Duties include comprehensive assessment, planning, and implementation of programmes of care, ensuring Mental Health Measure and care management protocols are adhered to and standards met. The coordinator will make evidence-based assessments of clients’ physical, psychological, spiritual and social needs, taking into account and respecting ethnicity, for a defined caseload.

• Significant Experience in Mental Health Services for older people
• Broad range of experience working in health and social care settings (Desirable)

• RMN Educated to Diploma Level

• Ability to prioritise and work proactively; manage a designated caseload without direct supervision.
• IT skills (Desirable)
• Phlebotomy skills (Desirable)
• Monitoring clients’ physical observations (Desirable)

• Ability to communicate clearly and accurately.
• Ability to adapt to new and changing circumstances.
• Self‑motivated.
• Commitment to continuous improvements to clients care (Desirable).

• Good understanding of mental health problems in the older person; awareness of current developments within a clinical setting; completed training in mentoring & assessing student nurses.
• Awareness of current developments within MHLD older peoples service (Desirable).
• Understanding of clinical governance (Desirable).
