Exciting Opportunity: Band 7 Clinical Pharmacist working in Mental Health Services.

Location: Leicestershire Partnership NHS Trust (LPT) pharmacy building in the grounds of Glenfield Hospital

Hours: Monday to Friday, 8:30 AM 4:30 PM or 9:30 AM 5:30 PM (On‑call duties as required) - Full Time (37.5hrs)

We are delighted to offer a fantastic opportunity for a Band 7 Clinical Pharmacist to join our dynamic and supportive Hospital Pharmacy team at Leicestershire Partnership NHS Trust (LPT).

This is an excellent role for a motivated and detail‑oriented pharmacist who is passionate about delivering high‑quality care. Main duties of the job

Job Description Summary (Main duties and responsibilities)

As a Specialist Clinical Pharmacist, the post holder will play a pivotal role within the pharmacy team, contributing to the delivery of a safe, effective, and patient‑centred pharmacy service across the Trust.

Key Responsibilities include:

a) Clinical Ward‑Based Services:

  • Delivering a high‑quality, patient‑focused clinical pharmacy service on wards, ensuring the safe and effective use of medicines.

b) Supporting Timely and Safe Discharges:

Facilitating early discharge and reducing hospital length of stay by:

  • Optimising patients drug therapy.
  • Providing tailored education on medicine use, including support with self‑medication and assessing the need for adherence aids.
  • Offering comprehensive counselling to patients about their medications prior to leave and discharge.
  • Ensuring continuity of care through robust discharge planning and communication with community healthcare providers.

c) General Pharmacy Duties:

  • Participating in wider clinical pharmacy and departmental responsibilities as required, supporting the overall objectives of the Trusts pharmacy service.
