A regional healthcare provider in Wakefield is looking for a member to join their Palliative care team to support daily patient management. The role involves coordinating referrals, booking outpatient clinics, and communication with patients and families. Applicants should possess a minimum of GCSE level education and NVQ Level 3, along with excellent communication and organizational skills. The post holder will support and assist the Palliative care team with the day-to-day management of patients that are referred to the SPCT. The post holder will support the CNS to triage referrals, booking patients in to the CNS diary, making direct contact with the patient and signposting where appropriate. The post holder will support and coordinate the running of outpatient clinics, booking clinic appointments. The post holder will enable and assist the CNS team in delivering high standards of care to all patients that are referred to the SPCT who have a palliative care diagnosis and may potentially be in their last year of life, they will support with low level early introduction to service visits. Part of the role will also be supporting the team by managing bookings throughout the trust for EoL mandatory training, support training where required.
