Overview

An exciting opportunity has arisen in Sefton Place for a Specialist Nurse Team Leader within the diabetes team. The postholder will operationally manage and lead the team whilst being accountable for the performance, quality and clinical effectiveness of the team. The post holder is expected to provide direct patient care when necessary, acting as an expert practitioner and a role model to the whole team whilst actively supporting integration. As the Specialist Nurse Team Lead you will work closely with the Integrated Community Nursing Senior Leadership Team to develop an effective and highly performing team, driving service improvements that meet patient need and promoting the team as a positive place for staff experience and development. The post-holder is responsible for the ongoing assessment of care needs and the development, implementation, and evaluation of care. The post holder will carry out all relevant forms of care and will take responsibility for caseload holders in their absence.

Details Date posted 17 September 2025. Band 7, Salary £47,810 to £54,710 a year. Fixed term 12 months. Working pattern: Full-time. Location: Sefton Place, Sefton, L21 9JN.

Responsibilities
  • Responsibility for service delivery within a designated area as a key aspect of the role.
  • Ensure the team functions efficiently and effectively, maintaining high standards of care and operational continuity.
  • Plan rotas, manage schedules, and adapt working patterns to meet service demands.
  • Collaborate with other services and stakeholders to facilitate seamless care transitions and optimised care pathways.
  • Deliver anticipatory and maintenance care, managing long-term conditions, promoting health and self-care, and providing end-of-life and palliative care as required.
  • Provide leadership and mentorship; support caseload management across the local health economy; mentor staff transitioning into these roles.
  • Act as an advocate for patients, challenging attitudes or behaviours that may compromise care quality or equity.
  • Provide clinical supervision and caseload oversight to ensure safe, effective, compassionate care.

Qualifications and experience

Essential

  • First level Registered Nurse
  • Evidence of post-registration education in related areas
  • Master's level study in a relevant area of specialist nursing

Desirable

  • Practice Teacher willingness to undertake
  • Previous management course/qualification
  • First level qualification in District Nursing / Specialist Practitioner qualification in District Nursing
  • Nurse Prescriber V150/300
