A healthcare organization in Stockport is seeking a Senior Community Clinician for a fixed-term role. The successful candidate will facilitate patient transfers and provide support within the Discharge Teams. Responsibilities include ensuring safe discharges and collaborating with multidisciplinary teams. The ideal candidate is a registered Nurse, Therapist, or Social Worker with experience in discharge procedures. Senior Community Clinician - Fixed term or Secondment post for 12 months
The post holder will work as part of the Discharge Teams to facilitate transfers from hospital and community beds, working in partnership with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers.
We are looking for a highly motivated and enthusiastic clinician who will provide professional and clinical support within the Discharge Teams and community beds as the service requires, which can include the Transfer of Care Hub, the Transfer of Care Team, Discharge to Assess, and the Intermediate Care at Home Team
This is a really exciting time to join our team to become part of our integrated service as we have many new and exciting initiatives to challenge and develop our staff .
The candidate should demonstrate strong communication/negotiating skills and be able to work closely with members of the MDT and other stake holders to facilitate timely and safe discharges to aid patient flow within the hospital and the community beds.
You will provide clinical leadership, expertise and advice to staff pertaining to all matters of discharge, quality and safe patient flow.
The key aims are as follows :
Operating hours are 08:00-18:00 seven days a week, this may increase to 20:00 in the future

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
