Qualifications

• Experience in a Reablement, Rehabilitation, Health or Social Care setting is preferred.
• General knowledge of the Care Act 2014 and Mental Capacity Act 2005 is preferred.
• Ability to assess risks, needs and analyse information.

Responsibilities

• Triage referrals and complete holistic assessments of need.
• Provide advice and information throughout the reablement journey.
• Build positive relationships with service users and their support network.
• Support safeguarding cases.
