Overview

We are seeking a proactive and experienced Clinical Pharmacist to join our multidisciplinary team. The successful candidate will play a key role in medicines optimisation, patient safety, and continuity of care, particularly in the context of long-term condition (LTC) management and polypharmacy.

Main duties of the job
  • Conduct comprehensive polypharmacy reviews to ensure safe, effective, and evidence-based prescribing.
  • Identify and implement alternative medications when standard options are unavailable, including liaison with specialists as needed.
  • Adjust prescriptions to alternative formulations e.g., liquid forms to meet patient-specific needs.
  • Provide cover for LTC pharmacist duties during periods of leave, including: reviewing incoming blood test results related to LTC reviews; actioning results in line with practice protocols and clinical guidelines.
