Maternal Child Health Nurse Team Lead

Department/Program: Health

Employment Status: Full-time, Permanent

1. Job Function/Purpose:

The Maternal Child Health Nurse Team Lead oversees the Maternal Child Health program as the Team Lead. The Team Lead is responsible for program development, budgets and supervising the team staff. The Team Lead ensures that all programs are relevant to the principles of the provincial Maternal Child Health Team.

2. Duties:

It is important to note that these represent the range of potential activities that can be carried out through this position. It is necessary to prioritize which activities will be provided based on community needs, worker time and existing resources.

  1. Abide by the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Code of Ethics.
  2. Abide by the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Confidentiality policy.
  3. Communicate and collaborate with co-workers, supervisors, and other staff, including team meetings, training sessions, case conferences, team building activities, performance discussions, day-to-day work and all other forms of written and oral communication using various mediums.
  4. Take part in evaluating program results and effectiveness (annually) through feedback from the community, maintaining and reviewing statistical data, keeping client records and files updated, and reviewing the Maternal and Child Health components of the Community Health & Wellness Plan.
  5. Conduct First Baby Visit assessments and monitoring of infants according to WHO schedule, referrals as needed.
  6. Deliver approved home care services (including, but not limited to) home visits, office visits, clinics, workshops, retreats, health fairs, family environments.
  7. Monitor the health of school age children through assessment and screening clinics.
  8. Promote wellness, nutrition, and healthy lifestyles to mothers and children 0-6 through personal visits, assessments and case conferences and respond as the individual situation requires based on the program mandate.
  9. Counsel families and children (0-6) in prevention, health promotion, of normal growth and development of children (ages and stages)- to connect with Aboriginal Infant Development Worker to assess children with development challenges.
  10. Provide information and advice to teachers, caregivers, parents, family support teams, addiction counselors, mental health counselor, physicians, clergy, elders, other helping professionals responsible for a specific client’s care.
  11. Organize and lead activities that encourage improved wellness, self-esteem, confidence building, healthy lifestyles and positive role modeling. Create fun things for families to do, develop a level of trust and rapport with young people and families to bridge emotional, spiritual, physical and mental balance in families and self.
  12. Encourage healthy parenting and child development from a Gwa'sala-'Nakwaxda'xw cultural perspective.
  13. Respond to the community members in need of support and assistance without delay organize and prioritize actions related to identified community priorities related to child and maternal health programs.
  14. Ensure all records and reports are prepared accurately, according to standard format, submitted on time and filed appropriately. Ensure all records and reports are filed in a timely manner and in compliance with the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Confidentiality policy and all relevant provincial and federal legislation.
  15. Ensure approved methodologies and approaches are adhered to in all aspects of practice and in accordance with the Community Health & Wellness Plan, the Gwa’sala-‘Nakwaxda’xw First Nation Personnel Policy, Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Code of Ethics, the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Confidentiality policy and all relevant provincial and federal legislation.
  16. Communicate and liaise verbally and in writing between clients, service providers, visitors, enquirers, and relevant staff, and interpret and respond clearly and effectively to spoken requests over the phone or in person, and to verbal written communication.
  17. Order and maintain relevant supplies.
  18. Engage the elder population of the Gwa’sala-‘Nakwaxda’xw First nation, work closely with elders and their wisdom and knowledge, be willing to do training and learn new skills, that are culturally sensitive to the Gwa’sala-‘Nakwaxda’xw First Nation people.
  19. Work with the Community Health Committee in a supportive way and provide organizational skills, advice, information, professional expertise as needed.
  20. Support and cover for other nursing positions when needed.
  21. Fulfill other duties as needed.
  22. To fulfill other related duties as directed.
3. Job Qualifications/Employment Requirements:
  1. Graduation from a recognized school of nursing, current practicing registration with College British Columbia College of Nursing Professionals (BCCNP).
  2. Strong interpersonal skills and ability to work as an effective member of a health team.
  3. Ability to adapt counseling services to community setting.
  4. Ability to work as a team player as well as independently in a one-to-one counseling environment.
  5. Possess problem solving and critical thinking skills.
  6. Knowledge of child development and intervention strategies.
  7. Knowledge and skill in organizational development and community health principles.
  8. Knowledge and skills in community development, social and political systems, community health issues, and First Nation history and socio-political issues.
  9. Working knowledge of Kwakwakwakw culture and the Gwa’sala-‘Nakwaxda’xw Health and Family Services, the community and their people.
  10. Three years’ experience in community health programming.
  11. Pass Criminal Record Check for vulnerable populations.
4. Working Conditions:
  1. On weekdays, some evening or weekend work may be required.
  2. Some travel may be required.
  3. The position adheres to the personnel policies and procedures, code of ethics and current job description of the Gwa`sala-`Nakwaxda`xw Nations.
