Job summary
A leading healthcare organization in Ontario seeks a Transitions Care Lead to facilitate patient transitions from hospital to home. The role includes managing critical relationships and ensuring quality client care. Qualifications
- Must be a registered member with the College of Nurses, Physiotherapists, Occupational Therapists, or Ontario College of Social Workers.
- 3+ years of recent experience in community health or related field.
- Knowledge of health care delivery system including hospital discharge planning.
Responsibilities
- Be the primary contact for hospital navigator/coordinator.
- Coordinate and review referrals for in-home transition services.
- Prepare initial care plans and monitor program metrics.
