Qualifications

• Must be a registered member with the College of Nurses, Physiotherapists, Occupational Therapists, or Ontario College of Social Workers.
• 3+ years of recent experience in community health or related field.
• Knowledge of health care delivery system including hospital discharge planning.

Responsibilities

• Be the primary contact for hospital navigator/coordinator.
• Coordinate and review referrals for in-home transition services.
• Prepare initial care plans and monitor program metrics.
