Do you have a passion for nursing? We have the position for you! We are seeking a motivated Registered Nurse with a dedication to delivering compassionate patient care to join our team in Primary Care.
In accordance with Northern Health's vision of an idealized system of services built on a foundation of Primary Care, where people and their families receive primary care services supported by interprofessional teams, the Primary Care Nurse (PCN) practices in accordance with the standards of professional practice and the code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM).
The Primary Care Nurse (PCN) is a member of the Mobile Primary Care Team delivering outreach to the Coast Tsimshian Communities in collaboration with community health professionals and teams. The role involves providing a continuum of services such as prevention, promotion, education, assessments, and care planning for individuals, families, and communities, along with consultation on cultural and clinical interventions.
The PCN functions as a member of the interprofessional team, applying best practices to provide competent, safe, and ethical care in settings such as physician practices, homes, schools, and communities. Responsibilities include clinical follow-up according to the person's primary Care Plan and personal goals of care, developing care plans collaboratively with the team, and providing direct clinical care to individuals, groups, and communities.
Primary Care Networks involve partnerships between the Divisions of Family Practice, regional health authorities, the First Nations Health Authority (FNHA), local community partners, and other providers such as nurse practitioners. These collaborations aim to redesign BC's primary and community care system for better patient and provider experiences.
This position requires travel and may involve hours adjusted around ferry schedules.
Shift Rotation/Hours of Work: Monday to Friday, Days, 08:30 to 16:30
