What You Will Do:
As a Referral Clerk, you will coordinate and process patient referrals by gathering and entering clinical and demographic information into the electronic health record. You will schedule appointments based on triage instructions and clinical urgency, and communicate with patients to confirm details and provide necessary information. You will support physicians by preparing documentation, initiating requisitions, and organizing referrals for review. You will also perform pre-registration tasks, assist patients with forms, and arrange for additional services such as interpreters or mobility support. Additional duties include maintaining statistical records, participating in quality improvement activities, and providing general clerical support.

Your Day to Day:
Receive and process referrals by collecting patient demographics and clinical data from physicians and hospital units.
Enter referral and appointment details into the electronic system, including consultation types, dates, and locations.
Register new patients and create agency identifiers according to established standards.
File and route clinical documents electronically for physician review and follow-up.
Prioritize appointments based on clinical urgency and organize referrals for triage.
Book new patient consultations and coordinate diagnostic imaging or additional documentation as needed.
Perform pre-registration tasks, confirm patient details, and provide appointment logistics.
Arrange support services such as interpreters, oxygen, or counseling based on patient needs.
Meet with patients to complete consent forms and follow up on missed or late appointments.
Maintain referral statistics, support quality improvement, and perform general clerical duties.
