An exciting opportunity has arisen for a registered nurse to join our friendly and forward thinking community care team in SMASH. This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The successful applicant will have a sound knowledge of recent changes within the NHS and knowledge of the political drivers to provide care closer to home.

As part of a team you will be responsible for the assessment of nursing care needs, implementation and evaluation of care programmes and the delivery of high quality holistic care within the community setting, and within our wound care clinics. You will be expected to support your peers, the team leader and the organisation in promoting the Trust’s values and behaviours.

The hours of work are between the hours of 08:00 and 17:00, over 7 days.

You will need to be able to work independently, possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas if required to meet service need. Please note whilst main base will be at Alsager, you will be required to work across the footprint of CCICP. Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.

We are looking for a Band 5 staff nurse to support the Caseload holders in delivering high quality, efficient and safe care. We are looking for an enthusiastic person who possesses both clinical and digital skills to support in the promotion of quality improvement for a local population.

In return you will benefit from a structured and robust induction package and the development of competencies within a very supportive team. You will also have an allocated mentor to work alongside you during the induction and probation process.

Responsibilities
  • To assess, plan and implement and evaluate a plan of care to meet the patient’s physical, social, emotional and environmental needs, encompassing a rehabilitative/self-care approach where necessary and as per district nursing contract.
  • To provide skilled nursing care and undertake clinical tasks including wound care, leg ulcer assessment and management, administration of medication via syringe pump, care/use of intravenous lines, venepuncture, and catheter care.
  • To be able to recognise, interpret and analyse clinical data related to the individual client’s needs including client observations.
  • To demonstrate, teach and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary.
  • To promote clinical excellence and evidence based approach to care by complying with the agreed Trust policies and procedures and maintaining clinical competencies in line with the competency framework.
  • To work to the End of Life (EOL) Strategy including adherence to all EOL tools, acting as a clinical expert in EOL care provision.
  • To take an active role in health education of the individual and community promoting self-care and providing both planned and opportunistic care based upon evidence based information.
  • To accurately record all patient contacts on Emis web and Trust documentation as per MCHFT policy ensuring patient and professional confidentiality is maintained at all times in accordance with NMC guidelines.
  • To manage and assess patients discharge potential and need from the caseload.
  • To ensure that equipment and aids, necessary for effective care, meet the needs of individual patients and are adequately maintained. To demonstrate and explain safe use of equipment for patient and/or carer where required.
  • To participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances.
  • To refer people to other practitioners when needs and risks are beyond one’s own scope of practice or require longer term support.
  • To prioritise and manage own caseload & client group.
  • To assume the responsibility for the clinical case load in the absence of the Team Leader/Senior Lead delegating work appropriately within the competency framework.
  • To participate in the measurement and monitoring of quality standards as appropriate.
  • To participate fully in the Non-medical Prescribing initiative if a qualified nurse prescriber, ensuring assessment of prescribing need is evidence based and cost effective and adheres to MCHFT wound care prescribing formulary.
  • To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.
  • To ensure the most efficient and cost effective utilisation of all resources (human, physical and financial).
  • To report accidents/incidents of patients or self which occur whilst on duty and to complete on the Incident reporting system immediately. To participate in any investigations as necessary.
  • To contribute to and co-operate in the coordination of off duty and holiday leave with colleagues, ensuring service cover is maintained. To work effectively to support other team members in all aspects of the service.
  • To maintain all aspects of MCHFT Integrated Governance Strategy and requirements including active participation in appraisal and clinical supervision in accordance with Trust policy.
  • To act at all times in a manner which illustrates respect for privacy, dignity and confidentiality.
Qualifications
  • Registered Nurse with current Nursing and Midwifery Council (NMC) registration.
  • Band 5 staff nurse level.
  • Full UK driving licence and ability to use a car.
  • Knowledge of recent NHS changes and political drivers for community care.

  • Optionally, qualification to act as a Nurse Prescriber for non-medical prescribing.
