Overview

We are very proud of our community nursing service and the difference it makes to people requiring nursing care in their own home. With an increasing complexity of patients in our community, our team provide seamless holistic nursing care to ensure patients receive the right care.

We are looking for individuals to predominantly work weekends across our teams and be part of our fast‑moving, innovative and ambitious service, dedicated to supporting our communities to manage their own health both at home and in our dedicated clinics.

Our community teams of Nursing and Allied Health Professionals deliver care to people who present with significant physical health care needs in various settings including clinic based, community inpatient, hospices to care in the patient’s own home, including care homes, enabling them to be independent, self‑caring and live their optimum lives, reducing hospital admissions.

Services range from community nursing, including specialist practitioners, to public health and health promotion such as school nursing and health visiting, with many specialist community services designed to support care pathways such as Physiotherapy, OT, Speech and Language, Dietetics and Podiatry.

Key Responsibilities
  • Provide skilled nursing care to patients across the locality and work flexibly with multi‑disciplinary teams to support our patients’ journey.
  • Provide appropriate community nursing care following a robust holistic assessment which enables patients to be cared for in their own homes and clinic settings.
  • Facilitate the prevention of unnecessary hospital admission/re‑admission by reducing the effects of ill health and disability and maximising the individual’s potential for independence.
  • Work flexibly in the community to support our evolving and developing services, supporting the team to deliver high‑quality integrated care.
  • Demonstrate excellent communication and interpersonal skills and be adaptable, reliable, resourceful and approachable.
  • Work independently while also being a team player.
Requirements
  • 5 posts, part time, 15 hours per week, predominantly weekend working; fixed term for 12 months.
  • Will start work at 8am and be available to work flexibly to meet the needs of the service, with the requirement to work between 8am and 12pm.
  • Suitable for nurses with community experience; transferable skills and students with a community management placement will also be considered.
  • Full UK driving licence and vehicle for business use required; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
  • Applicants will be required to pay for their DBS check (£26.40 standard or £54.40 enhanced); costs will be deducted from salary over the first 2 months of employment.
  • Applicants are encouraged to enrol for the DBS Update Service; an annual fee of £16 per year applies.
  • This post does not meet the pay level required for a Skilled Worker visa; applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. The Trust would not sponsor for a Skilled Worker / Health & Care visa.
