Job description

This is a new and exciting collaboration between Dementia UK and 6 Primary Care Networks in Sheffield . Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs. Based within multi-disciplinary/agency community teams and working alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a Clinical Network of Admiral Nurses .

The post holder will need to have the ability to work in a variety of settings.

Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities.

Please also note that in order to prepare you for becoming an Admiral Nurse, candidates are encouraged to complete the Pre-Admiral Nursing Module. This could be in advance of the interviews or upon an offer of employment being made. Details about this e-learning can be found on the Dementia UK website.

Main duties of the job

The Admiral Nurse will support, deliver andcoordinate relationship-centred dementia care within Dovercourt Surgery and the 6 PCNsinvolved in the project, in order toimprove the experience of carers/families affected by dementia withcomplex needs.

Inline with the Admiral Nurse Competency framework at a specialist level, thepost holder will provide:

Specialist bio-psychosocial assessment andsupport for people with dementia and families with complex needs

Facilitation of education and training for staffin the delivery of evidence-based dementia car

Support with policy development and qualityimprovements in relation to dementia care

Consultancy to other health and social careprofessionals regarding dementia

About us

Dovercourt Surgery is the host practice. This post will be delivering care across 6 PCNs in Sheffield.

Job responsibilities

Main Duties &Responsibilities:

Clinical

Adviseon the care of people with dementia and their carers/families with complex careneeds from peri diagnosis to post bereavement support

Providespecialist nursing assessment, using the Admiral Nurse Assessment Framework forcarers/families affected by dementia

Providea range of bio-psychosocial interventions to promote health and wellbeing forpeople with dementia and their carers/families

Workcollaboratively with other professionals to improve the identification andmanagement of co morbidities and frailty for people with dementia and theircarers/families

Developand support care plans for people with dementia and families including positiverisk management

Supportpeople with dementia, carers/families and staff to understand and respond tochanges in behaviour and relationships as a result of dementia

Providecare coordination for a clinical caseload of families with complex needs asidentified within triage assessment in clinics or via home visit if required

Ensurethe safeguarding of vulnerable adults and that all legal requirements relatingto Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), andDeprivation of Liberty Safeguards (2009)are considered and adheredto in relation to patient care

Actas an advocate for people with dementia and their carers/families to promote theirrights and best interests

Participatein case conferences and best interest meetings where required

Workcollaboratively with health, social and voluntary sector services to achievecoordinated care

Workin partnership with colleagues to promote the use of advance care planning forpeople with dementia and to support and inform families, around advance careplanning, best interest decisions and end of life care

Managecare transitions and support people with dementia and their carers/families toreduce the frequency of crisis situations

Ensurethat care is delivered in accordance with relevant local and nationalguidelines and all relevant Dovercourt Surgery (as host employer) and the 6 PCNs involved in the project policies.

Recognisethe limits of own competency and professional boundaries and make appropriateand timely referrals to other services where required.

Education & Support

Facilitate and support the development and delivery ofinnovative dementia training and education to staff, to improve competence and confidence, inline with best practice and guidance

Participatein and/or support formal training events promoting best practice in end of lifedementia care and disseminating Admiral Nursing work

Provide mentorship, offer clinical expertise and act arole model on the deliveryof evidence-based practice in dementia care within Dovercourt Surgery and the 6PCNs involved in the project.

Supportthe development and delivery of work streams as part of DovercourtSurgerys (as the hostemployer) and the 6 PCNs involved in the project dementia strategy, whereappropriate

Workproactively with key local and national stakeholders to support integrated carepathways and holistic models of care, through dissemination of specialistskills and knowledge

Supportdelivery of local policy and procedures for Dovercourt Surgery (host employer)and the 6 PCNs involved in the project to improve identification, assessment,ongoing support for people with dementia and their family carers.

Evaluation and Audit

Supportthe audit and evaluation of quality improvements and initiatives aimed atimproving the delivery of dementia care within Dovercourt Surgery and the 6PCNs involved in the project, where appropriate to the role

Collectquantitative and qualitative data to support the evaluation and audit of theAdmiral Nurse Service

Ensureinformation and data recorded is relevant, accurate, complete and captured in acontemporaneous manner

Professional

Ensure all nursing practice is carried out inaccordance with the agreed policies and procedures of Dovercourt Surgery (thehost employer) and the NMC Code

Attend and actively participate in practicedevelopment, clinical supervision, and continuing professional developmentsupported by Dementia UK and underpinned by the Admiral Nurse Competencyframework

Developown competence through use of the Admiral Nurse Competency Framework andportfolio development using resources provided by Dementia UK

Maintainprofessional registration in line with NMC guidance, including re-validation

Contributeto the development of policies, procedures and protocols relevant to dementiaand Admiral Nursing within Dovercourt Surgery (the host employer and the 6 PCNsinvolved in the project.

Ensurecompliance with the clinical governance requirements of Dovercourt Surgery(host employer) and the 6 PCNs involved in the project.

Maintainup-to-date knowledge of evidence-based recommendations on supporting peoplewith dementia and their carers/families

Maintainpositive working relationships with Dementia UK
