Job summary

Part time Advanced Clinical Practitioner £55,157 to £68,544 depending on experience/ KSF mapping.

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England.

We have a fantastic opportunity for an Advanced Clinical Practitioner to provide clinical leadership within a community-based multidisciplinary team (MDT) Vascular/Diabetes clinic across the Oldham area on a 12 months fixed term contract.

This service will consist of patients who have been referred by their GP due to suspected peripheral arterial disease (PAD). The patient journey will require a comprehensive vascular assessment, diagnosis (including ABPI) and a management plan, which may include a diabetic review +/- medication changes, lifestyle advice / health promotion and onward referrals where appropriate.

The successful candidate will be an experienced Advanced Clinical Practitioner with expertise in diabetes and/or vascular care. You will have a genuine passion for diabetes management and a commitment to delivering excellent patient-centred care within a primary care setting.

The hour of work for this post are 7.5 hours per week, Monday to Friday between 08:00 and 18:30.

Main duties of the job
  • Mobilisation of the well leg clinic, developing clinical pathways, standard operating procedures and assessment templates.
  • Supporting with training and undertaking of comprehensive assessments, diagnosis and management of PAD and diabetes management.
  • Providing holistic care to patients with diabetes, including monitoring, education, and support.
  • Leading, supporting and training junior staff with the MDT clinics and innovative ways in meeting individual patient needs (focussing on those that are underserved).
  • Collaborating with GPs, multidisciplinary teams and local services to optimise patient outcomes.
  • Promoting best practice in line within the current guidelines.
  • Providing data and analysis on specific patient outcomes and key performance indicators.

Job responsibilities

Clinical Practice

  • Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
  • Provide information and advice on prescribed or over-the-counter medication, regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Refer and discharge safely ensuring that safety-netting advice is offered.
  • Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Communication

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
  • Maintain effective communication within the organisation and with external stakeholders.
  • Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

  • Prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
  • Maintain knowledge and skills and prescribe within your scope of clinical competencies.
  • Comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

  • Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

  • One or both of the following: a Diabetic diploma or equivalent experience (including insulin initiation and titration) and a Vascular diploma or equivalent experience (including application and interpretation of APBI and ongoing management).
