Job summary
A local authority in Lancashire is seeking an experienced Adult Social Worker to join their team. This role involves triaging hospital discharges, applying Care Act eligibility criteria, and conducting Mental Capacity Assessments. The ideal candidate will have strong knowledge of adult social care pathways and excellent communication skills. This position offers flexible hours, a competitive pay rate of up to £33.00/hour, and aims to provide meaningful support to individuals transitioning from hospital to home.
Qualifications
- Experience in adult social care or hospital discharge pathways required.
- Strong knowledge of Care Act eligibility criteria needed.
- Ability to conduct Mental Capacity Assessments confidently.
Responsibilities
- Work within SPA to triage hospital discharges.
- Apply Care Act eligibility criteria.
- Complete Continuing Health Care Assessments.
- Conduct Mental Capacity assessments as per MCA 2005.
- Support transitions through hospital discharge pathways.

🕘 Hours: Mon - Fri | 09:00 - 17:00
💷 Pay: Up to £33.00/hour (Umbrella)
