Job description
Overview

Providing urgent assessments to those considered to have acute mental health concerns and offer robust home treatment options as an alternative to clinic appointments where necessary. This role is varied and challenging, offering opportunities to work in a variety of settings with people with a variety of mental health needs. You will work as part of a multi-disciplinary team to provide safe and effective treatment in the community, facilitate discharges from inpatient units to the community team and to primary care, and collaborate with service users, their carers and colleagues to positively manage risk in the CYP care pathway while enabling individual recovery journeys.

Demonstrates a high standard of record keeping and documentation in alignment with Trust policies. Delivers person-centered care reflecting current best practice and challenges practices that may be detrimental to older people receiving high quality care. Demonstrates a working understanding of the legal and ethical issues in mental health care, in particular knowledge of the Mental Health Act (1983) and its use in non-mental health areas. The post holder will work within the Community Specialist CYPMHS South team, building and maintaining positive relationships with individuals and agencies for effective collaboration and success. Clear communication, mutual respect and a collaborative mindset are essential for fostering positive connections with other agencies, including carers and service users.

These relationships are vital and team members should bear in mind that they represent HPFT. The post holder will build relationships with other parts of HPFT, notably the Children Crisis Assessment and Treatment Team (CCATT), Home Treatment Team and Forest House Adolescent Unit, as well as Hertfordshire County Council’s Health and Community Services. The post holder is responsible for the assessment of care needs, development, implementation and evaluation of care programs, setting standards of care and managing defined pathways. Staff supervision, development and teaching of staff and/or students, and working as a member of the multi-disciplinary team are expected. All staff should comply with the Trust’s Anti-Discriminatory Statement, Employee Charter, Trust Policies and Procedures, Code of Conduct and Equality and Diversity.

Responsibilities
  • Work as a member of the multi-disciplinary team in the community to provide safe and effective treatment and to facilitate discharges between inpatient units, community teams and primary care.
  • Collaborate with service users, carers and colleagues to manage risk within the CYP care pathway and support recovery journeys.
  • Maintain high standards of record keeping and documentation in accordance with Trust policies.
  • Deliver person-centered care that reflects current best practice and challenge practices detrimental to older people’s care.
  • Demonstrate understanding of legal and ethical issues in mental health care, including knowledge of the Mental Health Act (1983) and its application beyond mental health settings.
  • Build and maintain positive relationships with HPFT teams (e.g., CCATT, Home Treatment Team, Forest House Adolescent Unit) and with Hertfordshire County Council’s Health and Community Services.
  • Assess care needs, develop and evaluate care programs, set standards of care, and manage defined care pathways.
  • Supervise, develop and teach staff and/or students; participate as a member of a multi-disciplinary team.
  • Comply with Trust policies and procedures, Equality and Diversity, and the organisation’s values.
Qualifications
  • RMN or Dip/BSc (Hons) in Mental Health Nursing, ENB 812 or BSc in Specialist Community Nursing Practice (Mental Health).
  • Experience in psychosocial interventions and working in a range of settings relevant to the post.
  • At least 2 years of post-registration experience with at least 1 year at Band 5.
  • Experience in acute mental health inpatient/day care services, crisis and emotionally demanding environments.
  • Experience and knowledge of safeguarding processes, teaching and training, and working in a multi-ethnic community.
  • Experience of delivering evidence-based care.
Desirable criteria
  • Experience of working with acute mental health problems in a community setting and up-to-date knowledge of relevant legislation, ethical issues, strategic frameworks and current guidelines relevant to adult mental health.
  • Professional Code of Conduct; evidence of continuing professional development; clinical risk assessment and management in a community setting.
  • Ability to present using multimedia materials; knowledge of current developments in service user-focused research; skills to use and provide supervision effectively.
  • Skills in complex assessment and treatment for individuals, couples and families; ability to apply theory to practice and formulate community care plans; ability to communicate effectively within a multi-professional team.
  • Ability to work with diverse and inclusive communities across contexts; ability to provide and receive complex or sensitive information with empathy and reassurance; capability to manage stressful situations including safeguarding and risk of self-harm.
  • Ability to cope with verbal abuse and diffuse potential physical abuse; apply practice governance to maintain ethical and effective clinical practice in challenging environments.
  • Assessing and monitoring physical health care; substantial IT skills; ability to communicate complex information clearly to service users, families, carers and professionals within and outside the NHS.
  • Ability to work flexibly across service boundaries and to contain organisational stress; respectful approach to service users, families, carers, colleagues and other professionals.
Essential criteria
  • Ability to maintain high standards of record keeping, including robust case notes and electronic patient journey systems; ability to work autonomously and set goals in line with professional, ethical guidelines and Trust policies.
  • Confident to work autonomously in the community and use own initiative to make decisions in complex situations; ability to travel independently across the Trust.
  • You must have appropriate UK professional registration.
