Job summary
A local healthcare provider in Shepton Mallet seeks a compassionate Health Connector to join their team. The role involves coaching and motivating patients to achieve their health goals while delivering health education and facilitating social prescribing. Ideal candidates will hold relevant qualifications including 5 GCSEs and training in motivational coaching, and possess excellent communication skills. The position offers a salary of £12.75 to £15.08 per hour aligned with Agenda for Change scale 3-4.
Qualifications
- 5 GCSE at A to C or equivalent including English & Maths.
- Full clean UK driving licence is necessary.
- Degree or other qualification in relevant area or equivalent work experience.
- Training in health/motivational coaching and interviewing required.
Responsibilities
- Coach patients to set goals and achieve health objectives.
- Deliver health coaching support based on personal needs.
- Identify service gaps through patient feedback.
- Manage and prioritize patient caseload effectively.
