A local healthcare provider in Shepton Mallet seeks a compassionate Health Connector to join their team. The role involves coaching and motivating patients to achieve their health goals while delivering health education and facilitating social prescribing. Ideal candidates will hold relevant qualifications including 5 GCSEs and training in motivational coaching, and possess excellent communication skills. The position offers a salary of £12.75 to £15.08 per hour aligned with Agenda for Change scale 3-4.
We are looking for an individual with compassion andintegrity to join our friendly Health Connections team. You are likely to haveexperience in a community development role, whether paid or unpaid, in youthwork, adult health and social care, or advice and information services. HealthConnectors play an important role within our Primary Care Networkmultidisciplinary healthcare team.
You will coach and motivate patients through multiplesessions to identify their needs, set goals, and achieve personalised healthand care plan objectives, as well as facilitating groups offeringself-management education and peer support.
The Health Connections Team is part of (Central) Mendip PCN - which consists of 5 practices which have worked together since July 2019 - Mendip Country Practice, Coleford, Beckington Family Practice, Oakhill Surgery, Grove House Surgery and Park Medical Practice both in Shepton Mallet.
The PCN team consists of a Complex Care Hub - Lead Nurse and Care Coordinators, Frailty Team, Pharmacy Team, Home visiting Paramedics, Health Connectors, plus a Clinical Director, PCN Manager, Project Transformation Lead, and IT Transformation Lead.
Main responsibilities:Socialprescribing: helpingpeople to improve their health, wellbeing and social welfare by connecting themto community services. By increasing peoples active involvement with localdiverse communities, personal and community resilience is strengthened - andhealth and wellbeing inequalities can be reduced.
DeliverHeath Connections and PCN projects within the community, including TalkingCafes and healthy lifestyles education programmes, face-to-face or online asappropriate.
Identifygaps in service provision through speaking to patients and feed back to theSenior Health Connector or Manager.
Provideone-to-one and group health coaching support for registered patients, based onwhat matters to them, with the aim of: improving peoples levels ofactivation, empowering people to manage their own health and improvetheirhealth outcomes.
Manageand prioritise a caseload of patients. Understand when it is appropriate ornecessary to refer people back to, or on to, other healthprofessionals/agencies.
Help promote the Health Connections directory,resources, training, groups and one-to-one support across the locality.
Attendand contribute to team, practice and Primary Care Network meetings and eventsas required by the service.
Participate in regular supervision, reflective practiceand continual learning.
