Our Advanced Nurse Practitioners play a vital role as partof a busy team. In this position, you will play a pivotal role in providingexceptional patient care in a variety of settings, working alongside othermembers of the organisation or wider health and social care team. You will beresponsible for assessing, planning, and delivering care to patients acrossvarious age groups and clinical conditions, ensuring they receive the higheststandard of treatment and support.

Main duties of the job

  • Safelymanage undifferentiated and undiagnosed presentations using data gathering,assessing for red flags, investigation, diagnosis, management, and reviewwithin their agreed scope of practice and capabilities.
  • Mustdemonstrate complex clinical reasoning skills, to use problem solvingapproaches to analyse and synthesise information, manage medical and clinicaluncertainty particularly in relation to a wide range of contexts and clinicalpresentations, and to include physical, mental, and social problems.
  • Havea sound understanding of referral pathways including routine, urgent, 2-weekwait and emergency admissions.
  • Useresources appropriately and effectively.
  • Manageacute/chronic presentations and medical complexity.
  • Safelyand effectively use a range of consultation modes to include telephone, video,online and face to face.
  • Prescribetreatment and medication safely in line with local and national guidelines(where professional regulation and training permits i.e., an independentprescriber)
  • Tooffer personalised care to people and signpost to and link with other membersof the organisation or wider health and social care team.
  • Toprovide accurate, contemporaneous, and complete records of patientconsultations, consistent with legislation policies and procedures
  • Supportthe practices on-call triage system.

MainDuties & Responsibilities:

Work as part of themultidisciplinary clinical team, for the provision of health care services tothe patients of the organisation.
