Job Summary

Invicta Health in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT) are seeking to recruit Community Mental Health Practitioners to be part of Mental Health Together. Please note: this role does not include visa sponsorship; applicants must have the right to work in the UK.

What is Mental Health Together?

In Kent and Medway, the community mental health transformation model has created a new service called Mental Health Together. It is a multi-agency offer that brings all services and agencies supporting people with varying mental health needs closer together, so people get the right care without navigating service boundaries or repeatedly telling their story. Mental Health Together will add new roles as part of a joined-up model of care bridging gaps between primary care (GPs) and community mental health (KMPT). It aims to treat people with complex mental health needs by:

  • Providing someone with support in a person-centred, holistic way
  • Removing barriers to access and thresholds across services working together
  • Involving service users and carers as equal partners in their care
  • Building involvement and co-production at all levels of service design and delivery
  • Delivering needs-led and personalised trauma-informed care

Main duties of the job

The Registered Mental Health Nurse or Occupational Therapist will deliver stepped care by providing a range of evidence-based interventions to promote recovery, social inclusion and normalization for service users who have mental health problems. They will work in partnership with the MDT, CRHT, Acute Inpatient Services, other community teams, Primary Care and the Voluntary Sector to support service users and facilitate safe management of their needs within a range of community settings. The purpose is to promote mental and physical health and prevent ill health by delivering evidence-based interventions that build on strengths and support choice, underpinned by a single care plan within a model of care based on inclusivity, particularly for those with co-existing needs and marginalisation.

The post holder will be responsible for patients allocated to them and will be involved in assessment, management planning and delivery of care. They will support peers and team leaders, work as an effective team member, and supervise Band 5 Nurses/OTs and pre-registered students, holding a mentoring qualification. They will actively support and liaise with other health and social professionals.

Job Description – Main Responsibilities

  • Foster recovery through empowerment of service users and carers using a strengths-based approach; involve service users and carers in planning of their care; establish and maintain supportive relationships to encourage self-determination.
  • Offer a range of evidence-based interventions and risk management approaches to reduce and manage symptoms and high-risk behaviour during relapse.
  • Be proactive, innovative and creative in delivering care to facilitate service users’ recovery.
  • Engage service users and carers in relapse prevention and risk management or contingency plans to prevent admission or readmission.
  • Liaise with inpatient units to support discharge through appropriate pathways.
  • Facilitate early discharge from inpatients and the CRHT Team through regular reviews.
  • Deliver a flexible and responsive service aligned to service users’ needs.
  • Work collaboratively with the multi-disciplinary team and other agencies.
  • Deliver high-quality care and provide signposting to support correct pathway access.
  • Provide care within a Strengths-Based Model in the community.
  • Act as an educator for service users and carers to promote health literacy and self-management.
  • Respond swiftly to urgent referrals to prevent admission or readmission.
  • Work closely with carers; address health inequalities related to race, culture and sexual orientation.
  • Ensure accurate record keeping and data quality; use RiO for care planning and documentation as required.
  • Provide care plans in accordance with National Guidelines.

  • Communicate effectively with MDT regarding assessments, care plans, risk factors, relapse signatures and deterioration indicators.
  • Handle sensitive information professionally and communicate to identified persons only.
  • Respond to emergencies with clarity, sensitivity and direction.

Training

  • Act as a mentor to students; support their development and high standards of care.
  • Support students to meet learning needs and promote lifelong learning.
  • Plan and participate in mandatory training; undertake regular appraisals and personal development planning.
  • Support new staff integration and contribute to policy and guideline updates.
