Qualifications

• Eligibility to attain Australian Association of Social Work (AASW) membership.
• At least 3 years experience working with older people and their families.
• Knowledge of decision-making capacity and restrictive practices in aged care.
• Excellent verbal and written communication skills.

Responsibilities

• Collaborative management of significant and complex resident relationships.
• Providing guidance through periods of grief and adjustment.
• Assisting with SACAT applications including guardianship orders.
• Conduct psychosocial risk and capacity assessments.
• Navigating external health and welfare services.
• Care planning and advocacy for residents.
