It’s an exciting time to join us as we deliver the Heathier You NHS Diabetes Prevention Programme (NDPP) throughout the UK and we’re looking for remote Triage and Support Officers to join us.

The NDPP programme is designed to support individuals to make long term behavioural change in order to help halt or reverse the progression of Type 2 Diabetes over a nine-month period. As Triage and Support Officer, you will be the first point of contact for our clients, providing excellent customer service and helping individuals get enrolled on the programme.

Responsibilities
  • Delivering exceptional customer service whilst managing client referrals over the phone and electronically.
  • Inputting all client referral details into our database accurately.
  • Motivating service users to achieve their health outcomes, ensuring that all screening is carried out to the highest standards in accordance with NHS Code of Practice.
  • Working collaboratively with the NDPP team and partner organisations to create referral routes and help meet targets.
  • Prioritising key tasks depending on the needs of the service.
  • Working with colleagues across Thrive Tribe services to contribute to new projects and service developments.
  • Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
  • Helping maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and embedding our values.

Contract

Zero hours contract, bank/sessional staff.

Salary

£12.50 per hour base rate plus additional performance related bonuses.

Location

These are fully remote, home-based positions. All equipment required to carry out the role will be provided. The successful applicant will be required to have the right to work in the United Kingdom.
