Responsibilities

• Conduct comprehensive assessments for individuals ready to be discharged, ensuring safe and appropriate care plans.
• Collaborate closely with Occupational Therapists, service support managers, and a mental health liaison nurse to coordinate care.
• Facilitate the transition of service users to Ireland Lodge, a new reablement and step-down service aimed at supporting speedy and efficient hospital discharges.
• Develop personalised care packages or identify suitable placements tailored to individual needs.
• Monitor and review the progress of the service users, adapting care plans as necessary.
• Maintain detailed records and documentation in line with Brighton City Council policies and procedures.
