Overview

The Service Team Manager - Hospital Discharge Team role is a full-time, permanent position with hybrid working, and a salary of £53,296 - £58,286 per annum.

We have an exciting opportunity for a Service Team Manager to lead our Hospital Discharge Team!

Main duties of the job

As Service Team Manager for the Hospital Discharge Team, you will lead a multidisciplinary team working across acute settings to ensure safe and person-centred discharges. You will:

  • Provide visible, supportive leadership to staff, promoting resilience and reflective practice.
  • Coordinate effectively across health and social care partners to advocate for individuals rights and needs.
  • Ensure compliance with statutory duties including the Care Act, Mental Capacity Act, Continuing Health Care, and safeguarding.
  • Champion strengths-based and person-centred practice in a fast-paced, multidisciplinary environment.
  • Contribute to RBWM's strategic priorities: building inclusive, healthy communities, promoting prevention and choice, and delivering compassionate, high-quality services.
