Overview

Practice Nurse urgently required for a busy GP surgery in Reading. A great opportunity for an experienced Practice Nurse looking to work in a rapidly growing surgery in Reading. Hours: every Monday and Wednesday 08:00-18:00, starting ASAP.

Duties
  • Responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients.
  • Senior member of the practice multidisciplinary team, ensuring nursing services are delivered effectively to the entitled patient population.
  • Responsibilities across clinical areas such as infection prevention and control, health promotion, chronic disease management, well woman and well man clinics, and supporting the practice management team in reviewing and delivering clinical policy and procedures.
Essential
  • Registered Nurse with the Nursing and Midwifery Council (NMC)
  • Experience in chronic disease management (e.g., diabetes, hypertension, respiratory conditions)
  • Previous Practice Nurse experience
  • Qualified smear taker
  • Imms/Vacs trained, including childhood immunisations
  • Other typical requirements (e.g., DBS, right to work)
Desirable
  • Experience using EMIS
  • Diabetes qualification
  • Asthma/COPD qualification experience
  • Travel health
  • V300 prescriber
