Job Summary

We are looking for an enthusiastic Practice Nurse to join a high achieving team consisting of 6 GPs, 2 Advanced Practitioners, 3 Physician Associates, 5 Practice Nurses, 2 HCAs, with full admin support. The practice has an outstanding CQC rating (November 2018), a list size of 13,500 patients and uses EMIS Web for clinical records.

Salary is dependent upon experience and includes an NHS pension scheme and NHS holidays according to length of service.

Responsibilities
  • Supervise nursing assistants and health care assistants.
  • Provide nursing services (treatment, screening, preventative care, education and advice) in accordance with the NMC Code of Conduct.
  • Perform injections: childhood immunisations, vitamin B12, travel vaccinations, influenza, pneumonia and herpes zoster, following appropriate training.
  • Conduct cervical cytology screening following appropriate training.
  • Carry out basic treatment‑room duties, including removal of sutures, dressings, wound care and ring pessary changes.
  • Manage chronic disease under supervision, following appropriate training.
  • Perform New Registration Health Checks, CVD Health Checks and Over‑75 Health Checks.
  • Review patients on the mental health, dementia and learning disability registers.
  • Support public health and screening programmes, including immunisations and vaccinations.
  • Embed population‑health management approaches to target health inequalities and improve access.
  • Maintain medical equipment, emergency equipment and stock levels in partnership with the Practice Manager.
  • Ensure cleanliness, safety of working areas and proper disposal of clinical and hazardous waste in line with COSHH guidelines.
  • Assist in minor surgical procedures.
  • Accurate recording of consultations and treatments in EMIS Web.
  • Collaborate with other PHCT members and attend PHCT meetings.
  • Develop the role in line with QOF, IIF and CQC requirements.
  • Provide basic family‑planning reviews following appropriate training.
  • Train other staff, including pre‑registration student nurses and HCAs.
  • Maintain NMC re‑validation and undertake mandatory training and induction programmes.
  • Communicate with patients remotely and in person, including telephone consultations, face‑to‑face consultations and home visits.
  • Safeguard adults and children and adhere to practice policies and legislation on safeguarding.
  • Contribute to public health campaigns (e.g., flu clinics).
  • Keep the working area clean, tidy and effective at all times.
  • Provide cover for holidays and sickness and perform nursing‑assistant duties as required.
  • Work autonomously within the practice’s protocols.
Qualifications
  • Registered Nurse with valid NMC registration.
  • Evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
Desirable Qualifications
  • Mentor or teaching qualification.
  • Family planning qualification.
Experience
  • Broad knowledge of general practice and experience working as a Practice or Community Nurse.
  • Experience with chronic disease management.
  • Experience working autonomously.
  • Experience with infection prevention and control measures.
  • Experience managing elderly and housebound patients.
Desirable Experience
  • Experience of quality initiatives, e.g., benchmarking.
  • An appreciation of the NHS landscape including the relationship between individual organisations, PCNs and the commissioners.
Clinical Knowledge & Skills
  • Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and women’s health (cervical cytology, contraception etc.).
  • Request and process pathology tests and advise patients accordingly.
  • Understanding of evidence‑based practice.
  • Ability to promote best practice and work within own scope of practice.
  • Good clinical system IT knowledge and the ability to record accurate clinical notes.
  • Effective communication of complex and sensitive information by telephone, email and face‑to‑face.
  • Understanding of safeguarding adults and children.
  • Understanding of healthcare provision in GP surgeries, QOF and enhanced service.
  • Effectively able to understand patient needs.
Desirable Clinical Knowledge & Skills
  • Broad knowledge of clinical governance.
  • Knowledge of public health issues in the local area.
  • Awareness of national health promotion strategies.
  • Knowledge of national standards that inform general practice (e.g., NICE guidelines).
