Job description

We are seeking a compassionate, skilled, and dedicated Practice Nurse to join our team. As a valued member of our healthcare practice, you will play a key role in providing patient-focused care to individuals from diverse backgrounds. You will work closely with a multidisciplinary team to deliver high-quality care, health education, and support to patients, ensuring equitable access to services for all members of the community.

  • Delivering exceptional nursing care to patients and ensuring a high standard of service.
  • Leading and assisting with chronic disease management programs, including diabetes, asthma, and hypertension care.
  • Administering vaccinations (childhood and adult), immunizations, and necessary injections per protocols.
  • Conducting health screenings and assisting in early detection of illnesses through routine assessments and tests.
  • Promoting health education and awareness, tailoring advice to suit the individual needs of patients.
  • Supporting patients with family planning, sexual health services, and minor surgical procedures.
  • Maintaining accurate patient records and ensuring confidentiality by following data protection policies.
  • Collaborating with general practitioners, other nurses, and allied health professionals to provide integrated, team-focused patient care.
  • Auditing health services and participating in quality improvement initiatives to enhance patient care.
  • Acting as a resource for patient queries, providing advice, and directing patients to appropriate resources.
  • Upholding ethical nursing standards while demonstrating respect for diversity, equality, and inclusion in all aspects of patient care.
  • Additional training in specialist areas such as diabetes, asthma, or cervical cytology would be an asset.
  • Teaching or coaching skills to support health promotion initiatives.
  • Basic Life Support (BLS) certification or equivalent experience in emergency care situations.
Daily Activities
  • Conducting face‑to‑face consultations with patients and addressing their immediate healthcare needs.
  • Delivering routine procedures such as wound care, suture removal, blood tests, and ECGs.
  • Managing vaccination programs, including travel vaccinations and flu clinics.
  • Following up with patients to monitor and evaluate progress in managing chronic conditions.
  • Educating patients about maintaining healthy lifestyles and managing existing conditions effectively.
  • Working collaboratively with other team members to coordinate care plans and refer cases where needed.
  • Attending team meetings and engaging in discussions to improve quality of care.
  • Maintaining compliance with all nursing standards, procedural protocols, and ethical guidelines.
Qualifications
  • Registered Nurse (RN) qualification and current registration with the Nursing and Midwifery Council (NMC).
