A renowned medical practice in Nottingham is seeking a motivated Practice Nurse to join their compassionate team. The role involves providing care for patients, managing chronic diseases, and implementing treatment plans. The ideal candidate will have at least 3 years of post-registration experience and proficiency in cervical cytology. This position offers a chance to work with a multidisciplinary team focused on delivering high-quality patient care. Working hours are 20 hours over Thursday and Friday.

Qualifications

Job description

We are looking for a motivated Practice Nurse to provide care for the presenting patient, treatment, chronic disease management and evaluation of care.

Working in accordance with the NMC Code of Conduct, the post-holder will require experience in cervical cytology and baby immunisations essential. The post will play a crucial role in planning, co‑ordinating the work of the nursing team under the guidance of the Lead Nurse.

Main duties of the job
  • To develop, implement and embed health promotion and wellbeing programmes
  • To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
  • To identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to patients health
  • To maintain accurate clinical records in conjunction with current legislation
  • To ensure readcodes are used effectively by all members of the nursing team
  • To ensure the appropriate maintenance and use of chronic disease registers
  • To implement vaccination programmes for adults and children
  • To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • To support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Provide care for minor injuries, dressings, and higher‑level wound care, liaising directly, where appropriate, with community and secondary care counterparts, escalating to a GP or referring onto community services e.g., leg ulcer clinic/tissue viability etc.
  • Undertake opportunistic screening when clinically required e.g., blood pressure, BMI, smoking status, alcohol intake etc.
  • Perform venepuncture and electrocardiography and suture and staple removal
  • Assess problems presented opportunistically by patients

Experience
  • Of working in Primary Care.

  • Knowledge of evidence‑based practice and clinical governance.
  • Evidence of appropriate continuing professional development activity to maintain up‑to‑date
  • Knowledge and on‑going competence.
  • Evidence of working autonomously and as part of a team.
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice.
Qualifications
  • RGN with current NMC pin (if from a nursing background).
  • Evidence of continuing professional development.
  • Current NMC registration
  • Cervical cytology qualification
