Job description

City Health Care Partnership CIC (CHCP) is a co-owned organisation committed to contributing to the wider wellbeing of the community.

We are looking for an experienced, motivated and dynamic part-time Physical Activity Trainer/Yoga Instructor to join our enthusiastic and progressive team in delivering health services for people experiencing chronic pain.

The community based Chronic Pain Management Team provides an extensive service across Hull and East Riding of Yorkshire. CHCP Chronic Pain Management Service is committed to providing the highest quality of holistic care working in partnership with patients, their care givers and other health care providers.

Main duties of the job

You will work well as an integrated member of a multi-disciplinary team supporting service delivery of group and individualised treatment plans for patients in a variety of health care settings.

You will be expected to work under the direction of the Clinical Service Manager, delivering and supporting planned interventions with identified clients accessing Community Chronic Pain Management Services.

This will involve devising and delivering individualised or group, programmes specifically Yoga to patients experiencing chronic pain, with the aim of supporting the patient to become more physically active. The overall aim is to give support and encouragement leading to self management and independence, providing long term behaviour change with regards to increased activity levels.

You will be expected to work across the geographical area of service provision. You will support the individual within the remit of the contracted service specification; this includes maintaining a high standard of clinical data inputting for clinical records and service reporting purposes.

Person Specification
Qualifications
  • GP Exercise Referral Qualification (Wright Foundation/Gayton/YMCA/MSK) to level 3/4
  • Qualification in Yoga to level 3/4
  • Degree in exercise and health science or equivalent
  • Certificate in Helping People Change (or similar)
  • Completion of a lower back care course
Knowledge
  • Knowledge of exercise psychology and factors which influence exercise adherence such as motivation or the Stages of Change model
  • Knowledge of current physical activity recommendations and best practice
  • Knowledge of Long Term Health Conditions including Chronic Pain and understanding of the implications on an individual's ability to exercise
  • Knowledge of a range of medical conditions and medications and their relevance to exercise programming
  • Knowledge of current health agendas, strategy documents
  • Knowledge of Exercise Referral schemes.
  • Knowledge of a range of IT systems such as SystmOne or a willingness to learn to use such systems as an integral way of working
Experience
  • Experience of delivering Exercise Prescription for specialist populations
  • Working with individuals and groups promoting physical activity/health improvement behaviours to prevent /manage Long Term Health Conditions
