We are seeking a highly motivated and ambitious Part Time Pharmacy Technician to join our multidisciplinary practice team.

The practice recognise and value the attributes andskills that pharmacy professionals offer to General Practice and the successfulapplicant will complement the existing 2 Practice Pharmacy Technicians, Prescribing Clerk, ClusterPharmacist and Cluster Pharmacy Technician.

This is a fantastic opportunity for a PharmacyTechnician with aspirations to work within General Practice. You will join anestablished team and play an integral role in ensuring the practice prescribes safelyand appropriately for its patients, whilst being afforded the opportunity andsupport to challenge yourself in new clinical areas.

We are a large and busy GP practice, as such thesuccessful applicant must be able to work effectively under pressure,maintaining accuracy and quality throughout. The ideal candidate will be able todemonstrate a high level of counselling, interpersonal and communication skills.

They will have experience of working in a patientfacing role and possess the confidence and desire to keep evolving theirpractice, knowledge and skills.

Previous experience of working within a Primary care setting would be preferred.

Our Current Practice Partnership Consists of 1 Senior Partner GP,1 ANP Partner, 1 Gp Retainer, and 4 Salaried GP's.

The Clinical team Consists of 1 Advanced Paramedic Practitioner, 1 Minor Ailments Nurse,2 Pharmacy Technicians, 1 Prescribing Clerk,2 Practice Nurses,1 Healthcare Support Worker.Cluster team consists of 1 Cluster Support Pharmacist and 2 Pharmacy Technicians.

1 Practice Manager,1 Office Manager,1 HR/Finance Manager,1 Clinical Administrative Manager,1 Secretary, 2 Clinical Administrators, 1 Scanning Clerk, 6 Medical Telephonists.

Informal practice visits are welcome by appointment with the Practice Manager.

Job responsibilities

Main Duties andResponsibilities of the Job

Medicinesreconciliation post discharge and on other transfers of care, ensuringrecommendations are within local guidelines/formularies and not conflictingwith existing medication

Undertakeindividual assessments of patients and identify their medicines managementneeds. Ensure effective liaison with colleagues across health and social careto establish a suitable level of assistance

Undertakemedication use reviews, identifying and finding solutions for non-compliance

Demonstrate a sound understanding of the needs of frail,older and vulnerable people, including those with disabilities and promotetheir independence with regards to medicines management, advising on aids toindependence as appropriate.

Review and reauthorizerepeat prescriptions within own competence and as per practice protocols,arranging any necessary monitoring

Dayto day handling of prescription queries within area of own competence, from patients,carers, care homes, pharmacies, hospital teams etc.

Assess the suitability of requests for acute prescriptions within own competence and per practiceprotocols

Targetedmedication reviews in line with practice protocols

Counsellingpatients on new medicines, such as DOAC initiations

Managecomplicated medication regimes, such as dose titrations and cross taperingregimes, ensuring patients manage these safely through counselling

Identifysuitable alternatives for medicines that are unavailable

Manage serious out of stock notices proactively toavoid any delays or breaks in therapy for patients

Thepractice provides a Pharmacy Technician led INR clinic service and the successfulapplicant will have the opportunity to receive training in order to deliver this clinic

Provideeducation to patients and their carers with regard to medicines management,including their storage, ordering, administration and identification ofpotential problems such as side effects

Workclosely with palliative and community nurses to ensure requests for medicationare dealt with in a timely manner.

AssistGPs in prescribing syringe driver and just in case medication

Deliveryof training and development to practice based prescribing clerks andadministrators to support improved co-ordination and effective pharmacyadministration within General Practice

Provide direct support and advice to GPs, ANP andother health professionals to promote and implement high quality prescribing tothe benefit of patients

Promote cost-effective, safe,evidence based prescribing in accordance with national and local guidance

Work closely with HealthBoard and Cluster Pharmacists/Pharmacy Technicians

Participate inpractice meetings and contribute to any prescribing issues

Present to clinicalstaff updated guidelines and feedback on external meetings (e.g. prescribingleads meetings)

Contributeto the review and development of repeat prescribing protocols / StandardOperating Procedures to improve the efficiency and effectiveness of repeatprescribing systems

Liaise with otherpharmacy stakeholders e.g. Medicines Management team, community pharmacies andhospital pharmacy teams.

Liaise with community pharmacies regarding theprovision of medicines administration charts, compliance aids, medicationreviews etc

Reviewprescribing in line with National Prescribing Indicators and local PrescribingManagement Scheme

Handle information whichmay be sensitive, complex or confidential appropriately, in accordance with theData Protection Act and local information governance policies

Be responsible for managingown workload and prioritising as necessary

Utilise GP practice clinicalsystems / Health Board systems to conduct prescribing audits

Undertake, participate andshare the outcomes of clinical audits to facilitate improvement and changes inpractice

Ensure that all major MHRAdrug alerts, where appropriate and within levels of competence, are acted uponwithin an appropriate time frame and to advise on any medication changes thatoccur as a result

Undertake training anddevelopment, to ensure the required knowledge and skills for practice basedwork

Carry out other duties whichare appropriate to the skills and competencies of a Pharmacy Technician as thepriorities of the service change
