BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a part-time outpatient home health position. The role involves providing home visits for senior clients at our Harrisburg, PA office, serving adult and geriatric populations across York County.

Applicants must have at least one year of clinical experience as a licensed Physical Therapist. The position offers an opportunity to work closely with clients, focusing on rehabilitative care and skilled nursing to help them remain at home.

Responsibilities include:

  • Visiting clients in designated territories.
  • Performing diagnostic tests such as mobility, strength, balance, and safety assessments.
  • Developing and implementing individualized care plans with therapeutic exercises, gait training, and other interventions.
  • Revising care plans as needed and collaborating with nursing staff.
  • Educating clients and families on rehabilitative activities to promote health and independence.
  • Documenting care activities accurately using electronic tablets.

Qualifications:

  • Current PA Physical Therapist license.
  • Minimum one year of recent PT experience.
  • Graduation from an accredited PT program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal and organizational skills.
  • Proficiency in computers; experience with EMR preferred.
