The Managing Memory Service Team, provides a specialist service for people with memory problems / dementia across Gloucestershire.
This role is key in coordinating the on-going assessment, treatment and support to people with dementia, their carers and relatives who are referred to the memory service.
To work collaboratively with members of the memory services team maintaining professional links to ensure that the service user and carer get the best service. To undertake assessments and treatment with people with memory problems / dementia, their carers and relatives who are referred to the memory service, and to lead the provision of post diagnostic discussion.
