An exciting opportunity has arisen for a dynamic Nurse to join the amazing Poole Intermediate Care Service. We are a multidisciplinary Team who prevent hospital admissions and secure early hospital discharges.

We deliver a hospital at home service, caring for patients in the comfort of their own homes.

We have full time hours on offer but part time hours or a job share could be considered. The post holder may be required to work flexibly to meet the needs of the service.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

Responsibilities
  • Demonstrate leadership.
  • Provide a high standard of holistic assessment and appropriate timely reviews.
  • Plan specialist care and/or rehabilitation programme, to promote and sustain independence/well-being for the individual in their own homes.
  • Deliver nursing, rehabilitation and care programmes for adults, many of whom have complex clinical and social needs with the aim of improving independence, function and quality of life.
  • Meet Enhanced Disclosure and Barring Service (DBS) check requirements; the Trust will cover the cost of the check. DBS‑update subscription enables applicants to present a valid DBS certificate instead of a new check.
