A local government authority in Merthyr Tydfil is seeking a Senior Practitioner to oversee a team supporting adults post-hospital discharge. The role involves assessing needs, developing support plans, and supervising social work staff. Successful candidates will need to provide evidence of qualifications, and Welsh language skills are desirable. The council values diversity and encourages applications from all backgrounds.
This is a newly formed team in Merthyr County Borough Council that has been established to meet the requirements of the Discharge to Assess policy and to provide effective, seamless support to adults who are being discharged from hospital. This will involve supporting the person for up to 8 weeks post discharge to ensure there is an effective plan in place that reduces the risk of re-admission to hospital.
