Job description
Overview

This is a newly formed team in Merthyr County Borough Council that has been established to meet the requirements of the Discharge to Assess policy and to provide effective, seamless support to adults who are being discharged from hospital. This will involve supporting the person for up to 8 weeks post discharge to ensure there is an effective plan in place that reduces the risk of re-admission to hospital.

Responsibilities
  • Assess people in hospital/being discharged, identify the package of support required to move them back into the community safely, and develop the plan to support this and review whether the plan is effective in relation to prevention of re-admission.
  • As the Senior Practitioner in the team, provide supervision and daily support to two Hospital Discharge Social Workers and one Assessment and Review Officer and ensure the effective functioning of the team.
