A healthcare organization in England is seeking a dedicated Health Care Assistant responsible for delivering quality care to individuals in their homes. The role involves collaboration with community nursing staff, flexible working hours, and the requirement of a car for service duties. You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.

The Community Nursing Service operates between 08:00 and 21:00 hours, and the working pattern will include a variety of rotational shifts which will include weekend, evening and Bank Holiday working. Part-time posts would be considered.
