We have an opportunity for 1 x full-time GP Assistant based at Royal Primary Care Brooklyn, Heanor. Royal Primary Care Brooklyn is seeking an experienced healthcare assistant with an interest in frailty to support our multi-disciplinary team of professionals – including working alongside our colleagues in the ARCH PCN (Alfreton, Ripley, Crich and Heanor) and other stakeholders in the community. RPC Brooklyn is ideally situated between Derby and Nottingham and has been serving the healthcare needs of the local community for over 120 years.

This is a developing role within our practice team providing care to vulnerable, frail and housebound patients. This role will also involve working alongside GPs and Advanced Clinical Practitioners aligned to care homes, supporting with HCA tasks, the weekly ward round and annual long-term condition reviews as required.

Responsibilities
  • Deliver care to vulnerable, frail and housebound patients within the practice and care homes as part of a multidisciplinary team.
  • Contribute to the weekly ward round and assist with annual long-term condition reviews as required.
  • Assist GPs and Advanced Clinical Practitioners in care home settings and in the community.
  • Undertake a diverse range of assessments and provide appropriate care/treatment according to patient needs, aiming to improve standards of care and enhance nursing services within the practice.
  • Support with HCA tasks as part of the care team.
  • Potentially contribute to monitoring Long-Term Chronic Disease management of patients, dependent on experience.
Qualifications / Experience
  • Experience of providing care and support to people in primary care, the community or care homes.
  • Interest in frailty and ability to work collaboratively within a multidisciplinary team.
  • Knowledge of the GMS contract (e.g., The Quality and Outcomes Framework) would be valuable.
