Job description
Overview

Dietetic Assistant Practitioner, Band 4. 26 hours per week. Fixed term until 2 October 2026. We are looking for an enthusiastic Band 4 Dietetic Assistant Practitioner to provide maternity cover. The post is part time 26 hours per week and is part of the community Dietitian Team. The role will assist the registered Dietitians with the assessment, implementation and monitoring of nutritional care plans for adults referred to the Service.

You will spend time with patients and carers to optimise patients' nutritional intake and reduce the risk of malnutrition. The post is community based and you will see patients on the intermediate care unit as well as in care homes.

Responsibilities
  • Independently undertake nutritional screening/assessment of appropriate patients once triaged by the Dietitians.
  • Support the dietitians by reviewing and monitoring oral nutrition support patients in a range of community settings, independently amending care plans.
  • Develop nutrition care plans for appropriate patients and be responsible for carrying out reviews in a timely manner.
  • Work independently whilst being aware of your own competencies.
  • Liaise with the Dietitians and work within competencies, transferring complex patients to the Dietitians when needed.
  • Independently prioritise patients within your caseload.
  • Have a working knowledge of nutritional supplements and request prescriptions appropriately.
  • Liaise with staff (nursing, carers and catering) in care homes regarding a patient\'s nutritional progress and feed back to the dietitians as appropriate.
  • Interpret food/fluid record charts and weight charts and make appropriate suggestions to patients/nursing staff if changes are required to the care plan.
  • Calculate body mass index (BMI) and interpret the result, taking appropriate action.
  • Order nutritional supplements for the intermediate care unit, ensuring sufficient stock to meet patients\' needs.
